Service Description: 123-bed acute and continuing care hospital with emergency department * services include medical, surgical, complex continuing care, maternal child, mental health, critical care and integrated stroke unit * Care Coordinators from the Home and Community Care Support Services South West Region available on site

Huron Perth Diabetes Education Programs - Supports for people living with prediabetes or diabetes * one-on-one consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff
 
Huron Perth District Stroke Centre - Integrated stroke care * information and resources for stroke survivors * promotes healthy living and stroke prevention with community partners
 
Maternal Child Unit - Multidisciplinary team of obstetricians/gynecologists, pediatricians, midwives and health care providers

Application: Walk in for emergency department * medical referral required for most other services
