	Community Connections St Thomas and Elgin County 2019 A resource guide to basic needs supports and services in St Thomas and Elgin County.
	Coordinated Care Planning The Coordinated Care Planning approach intends to improve communication and collaboration among providers who share in the care of people with high care needs. Multiple providers, appointments and complex care issues can make it difficult to meet these individuals' needs. A more collaborative approach to providing care can be achieved through a process called Coordinated Care Planning.
	Exercise and Falls Prevention Classes in the South West Seniors in the South West now have improved access to exercise and falls prevention classes. The South West Local Health Integration Network (LHIN) selected a number of Community Support Service providers to offer these classes throughout the region. Check back regularly as more locations are added.

	Interprofessional Comprehensive Geriatric Assessment Toolkit This toolkit provides foundational information and learning resources related to Comprehensive Geriatric Assessment.
	Joint Replacement Resources Provides resources that will help prepare people for what to expect before, during and after joint replacement surgery, including access to patient guides and community resources within the South West Local Health Integration Network (LHIN) region of Ontario.
	Mental Health at School This website is a community resource created through the Mental Health and Addictions Nurses (MHAN) initiative to help teachers, students, parents, and providers find local services that can help.
	Ontario Telemedicine Network Optimization A collection of practical resources, tools, videos and how to’s are available to help you and your team boost your OTN skills. This collection of resources is relevant to all health care sectors to enhance OTN use for meetings, educational and clinical events.
	Rehabilitative Care in South West Rehabilitative Care in Ontario (rehabcareontario.ca) is a provincial website that makes it easier for you to find rehabilitative services in your area offered by organizations that are publicly funded and are provided/supervised by regulated health professionals (health professionals with special training such as physiotherapists, physiatrists [rehab doctors], occupational therapists, speech and language pathologists and others).
	School Health Support Services The South West LHIN's School Health Support Services (SHSS) provide health care services to children who otherwise would be unable to attend school, or whose education would be disrupted by having to leave the school setting for needed therapy.
