Service Description: Network of hospitals, universities and colleges, community and research organizations, predominantly in the South West and Erie St Clair regions, that are committed to improving population health in Southwestern Ontario

Project priorities are guided by a Network Contributors' Roundtable, which is comprised of leaders from each of SWAHN's financial contributors * project streams align with the Network's value proposition in the areas of Interprofessional Collaboration, Knowledge Generation and Translation, and Networking and Engagement * project areas include: palliative care, interprofessional education, nutrition and "Choosing Wisely"
