Service Description: Central Intake service offered by Community Support Providers in London and Middlesex. Service includes information about existing community support services and how to register and access these services. CSS Central Intake is able to connect individuals to multiple community support services through a single intake

Meals and Nutrition - Community dining, Meals on Wheels (hot and frozen)

Education and Support - Adult day program, individual and group supports, caregiver supports, education, information and system navigation

Safety and Reassurance - Visiting and telephone reassurance, education and behavioural supports, support on hospital discharge, and personal emergency response systems

Health and Wellness - Social programs, group fitness, self-management, bathing, health education, falls prevention, medication management supports

Support in the Home - Personal supports, housekeeping, in-home meal preparation, respite, and caregiver relief

Transportation - Rides to medical and wellness appointments. Rides for social activities, errands, and shopping may be available in some instances

Intensive Support Programs - Assisted living, adult day programs, overnight and day respite, and supportive housing, senior’s wrap-around program

Specialized Services - Supports for people with memory loss/dementia, acquired brain injury, adults with disabilities and their caregivers

Fees: Varies per program/service
Application: Call, no referral required * client residence and services needed will determine which agency delivers service
Eligibility / Target Population: Adults 55 years and older and/or adults under 55 with a disability, dementia, acquired brain injury or palliative * supports are also available for caregivers
