Service Description: Community program that supports older adults to age in place through the implementation of the Elgin St. Thomas Age Friendly Community Plan. Works with community partners to reduce risk factors and increase protective factors that contribute to injuries

Application: For clinical assessments apply through Home and Community Care Support Services South West Region or health care provider
Eligibility / Target Population: Older adults, seniors, frail elderly, caregivers for older adults/elderly
