Service Description:

Palliative, respite and transitional health care services for those with HIV/AIDS/HCV in a home-like environment * services include case planning, social and recreational support, nutritional services, house keeping, and community coordination
Clients are admitted for a maximum of 12 months * short-term and respite stays available

Set fee for Hospice and Respite Care * fee is based on ability to pay

Eligibility / Target Population:

People who are HIV positive and have an HIV related illness (not necessarily an AIDS defining illness) and people with Hepatitis C
