Service Description: Refers people who do not have a doctor to a family health care provider who may be accepting new patients * once registered, program participants are assigned to a Care Connector employed by the Home and Community Care Support Services  * Care Connectors work with family doctors and nurse practitioners who are accepting new patients


Fees: Covered by OHIP
Application: Register online or by phone
Eligibility / Target Population: People of all ages who have a valid OHIP card and are without a regular health care provider * must complete a health questionnaire to determine their need for family health care services * priority given to people who have greater health needs
