Community Care Access Centres - London/Middlesex

The Community Care Access Centre (CCAC) helps the elderly, people of all ages with disabilities and those who are ill to remain in their own homes for as long as possible. When living independently is no longer a safe option, the CCAC assists individuals and their families gain access to a long-term care home. The CCAC ensures quality health care to school-age children who have special medical needs and helps people to return to their homes by providing quality home care after surgery or a hospital stay.

To access services offered by the CCAC, contact your local office, or contact any CCAC office where your call will be directed as required.
