Rehabilitative Care - In-Home - South West

Rehabilitation services provided in the patient's home or other non-clinical locations. Services can include physiotherapy, occupational therapy, speech language pathology, case management, and vocational or other evaluations. Publicly funded in-home rehabilitation services may be arranged by Home and Community Care Support Services.
