Service Description:

Rehabilitative nursing and case management for children, adolescents, adults and seniors with a physical disability or medical diagnoses, have sustained a traumatic injury (e.g. brain injury) and experience challenges relating to physical, cognitive, social, emotional, and/or behavioral functioning
School integration consultation - information, education to school personnel and advocacy for children, adolescents and young adults within the school systems, including postsecondary * advocacy for appropriate accommodations and modifications
