Service Description: 20-bed acute and complex continuing care hospital with emergency department * Care Coordinators from the Home and Community Care Support Services South West Region available on site

Complex Continuing Care - 10-bed rehabilitation program to help medically complex patients maintain their strength and movement * access to nursing care, therapy, and weekly follow-ups with a doctor or nurse practitioner
 
Huron Perth Diabetes Education Program - Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff
 
Outpatient Clinics - Outpatient specialist clinics providing diagnoses and treatments for various medical conditions

Fees: Most fees are covered by OHIP
Application: Walk in for emergency department * medical referral and appointments required for most other services
