Provides counselling services and family support services
Adult Protective Services - case management involving assessment, coordination, advocacy and counselling for individuals 18 years and over who have a developmental disability
Counselling Services - individual; couples; family therapy; group therapy; seniors caregiver support and counselling; family violence treatment program; men's groups; women's groups
Credit Counselling - includes personal money management, budget counselling, arrangement and administration of the Consumer Debt Repayment Program, promotion of consumer credit education and mediation services * www.debtontario.com
Employee Assistance Program - contracted service through employer offering employees help with personal concerns, marital issues, family troubles, parenting, stress management, alcohol or drug abuse, financial problems, job related problems, separations or divorce, eldercare, grief, coping with illness or injury, physical or sexual abuse and depression
Family Support - Case Management - provided to families with a child (birth to 18 years) who has a developmental or physical disability * information on referrals and assessment to determine appropriate needs, coordination of available resources and counselling to the individual and family or caregiver * advocacy is also provided
Family Violence Program for Men - group treatment for male perpetrators of domestic violence (Partner Assault and Child Abuse) * sixteen week group, psychoeducational format
Foundations Initiative - provides young people 18 to 24 years with developmental disabilities the opportunity to develop leadership, literacy and numeracy skills as well as recreational, leisure and social opportunities * short term respite services are also available
Hospice Services - provides compassionate care in home or out of home through services provided by trained volunteers * information and support is provided to individuals with life threatening conditions, their families and caregivers in cooperation with others * bereavement support for anyone who has experienced the death of someone close
Literacy/Numeracy - classes provided to adults with developmental disabilities on a one-to-one basis or in small groups who require skills for a job or volunteer position
Senior's Program - information and service to seniors and families with regard to decisions in changing accommodation and legal and financial issues and counselling * additional services include advocacy, public education workshops and presentations, counselling to caregivers and children of seniors * elder abuse assist and referral
Social Work - individual and family counselling and advocacy * short term basis to clients referred by the South West Community Care Access Centre - Seaforth and Stratford offices
Special Services at Home - provided on a one-to-one basis to infants, children and adults with a developmental disability * applicants must be living at home with a family member to receive service * provides support through teaching of daily living skills, community integration, behaviour management, social skills and the implementation of programs designed by physiotherapy, occupational, speech and infant therapists * offers respite
Familyhome Program - housing program for people with a developmental disability * located in Listowel
Respite Program - short-term respite care services for children with complex medical needs
