Service Description: The eShift model of nursing care utilizes specially trained Personal Support Workers (ePSWs) who receive support from an off-site, directing nurse through a Smartphone application. The ePSW carries out activities on behalf of the directing nurse while providing care for clients, allowing clients to stay at home. The eshift program is for patients with chronic or life-limiting illness with a significant trajectory.

Hospice at Home utilizes the eshift model of care, but is specifically for patients with an end-of-life trajectory, allowing patients to palliate at home. As a dedicated capacity funded model, there are a set number of funded beds established in each region throughout the South West.

Fees: Covered by OHIP
Application: Call * no referral required for most services
Eligibility / Target Population: Eligibility - Care Coordinator will outline service to client/caregivers and establish eligibility and consent. Target Population -eShift – chronic palliative and complex; all ages -Hospice at Home – adults 18 and over, palliative w/ an end-of-life trajectory of 2-3 months
