Service Description: 6 month telephone coaching, support and monitoring program patients with mild to moderate Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF) * equipment provided (blood pressure, weigh scale, pulse oximeter and tablet) to report on health daily * weekly discussions with telehomecare nurse
