Service Description: Supports for adults, children and families living with a developmental disability
 
Adult Protective Services - Supports adults with a developmental disability who live or are planning to live independently in the community * assistance is provided through planning, coordination and case management of community resources, advocacy, facilitation of community access and inclusion, monitoring of supports, resource information and connections, community development and education
 
Buddy Up Social Skills Group - Facilitated group for youth, ages 11 to 14, with Autism Spectrum Disorder (ASD) * supports young people in learning and practicing social skills with peers, developing self-confidence
 
Family Support - Case Management - Supports families caring for a child with a developmental disability, providing emotional and instrumental counseling, learning about service options, completing funding applications, problem solving, fostering community and service connections and supporting family interactions with professionals, agencies, systems and others
 
Familyhome Program - Supported private home placement for adults with developmental disabilities
 
Foundations Initiative - Provides people 18 to 26 years with developmental disabilities the opportunity to develop leadership, literacy and numeracy skills as well as recreational, leisure and social opportunities and community participation
 
Passport Program - Helps adults with a developmental disability be involved in their communities and live as independently as possible * supports for the activities of daily living and person-directed planning * funding for community participation services * funding for caregiver respite services and supports for primary caregivers
 
Special Services at Home - Provides a variety of respite and in-home support for families that care for an infant, child or youth (to age 18) with a developmental and/or physical disability * a trained service provider is matched with each family
