Service Description: Stroke recovery adult day program in collaboration with St Joseph's Health Care Community Stroke Rehabilitation Team and Woodstock Hospital Intensive Rehabilitation Outpatient Program * program focuses on regaining skills necessary for activities of daily living

Overnight Respite Available at Woodstock Site for clients throughout Oxford County * offered most Tue and Wed * room for 2 clients * 2 PSW staff * RPN support can be arranged for as needed * disscuss with coordinator upon referral

Health Care Assistance
Medication Assistance Assists clients with medications when required
Mobility Assistance Staff trained in transfers, escorting and falls prevention * clients with canes, walkers, wheelchairs etc, accommodated * tub room and washrooms are accessible
Incontinence Provides support to clients with bladder incontinence * clients are encouraged to wear briefs or pads to support bladder control * a spare change of clothes can be stored in the program
Bathing Bathing program available for clients and members of the community (weight restrictions for tubs and lifts)
 
Other
Transportation Family encouraged to provide transportation to and from program * staff can help arrange transportation if needed
 

Fees: $11 per day, plus additional fee for bathing
Application: Referral from Community Stroke Rehabilitation Program or Intensive Rehabilitation Outpatient Program
Eligibility / Target Population: Recipient of the Community Stroke Rehabilitation Program * requires ability to actively participate
