Organized stroke care across the continuum of care
Regional Stroke Centre - Leadership, development, implementation and coordination of stroke care throughout the region * includes health promotion, primary and secondary stroke prevention, acute stroke care, rehabilitation and home care * professional education/training in stroke care
District Stroke Centres - Integrated stroke care to patients within their facility and their catchment region
Stroke (Secondary) Prevention Clinics - Stroke prevention for patients within their community, who are at high risk for stroke, have had a Transient Ischemic Attack (TIA) and/or who have had a stroke
Stroke Resource Page - A resource for people recovering from a stroke and their families and caregivers
