Service Description:

Partners from across the continuum of care work to address the needs of the community and older adults at various risk levels of falling
Services include:
- fall prevention awareness and education
- program planning and policy development
- development and distribution of fall prevention assessment tools and resources for older adult community members and healthcare professionals
