Service Description: Community health care agency * Personal Support Managers available to complete free in-home assessment to establish care plan that meets patients needs to remain independent in own home.

In-Home Personal Support Workers provide:
  • Bathing/assistance with personal care
  • Dressing assistance
  • Mobility assistance
  • Meal preparation assistance


Nursing services are provided in designated clinic locations. Please call our office for further information.  Clinic nursing services include:
  • Wound care
  • IV Therapy
  • catheter
  • Drain/staple/suture care
  • Health teaching regarding diagnosis/disease self-management/medication management

Nursing visits in-home are provided for patients who meet the clinic exclusion criteria.
