Aboriginal Midwifery - home births and visits in London, Chippewas of the Thames, Oneida Nation of the Thames and Munsee-Delaware Nation
Adult Mental Health and Addictions Services - Counseling, support and transitional case management for Indigenous adults * addictions counselling * crisis intervention, planning and prevention, assessments and screening * Teaching Circles programs covering topics including self-care, identity and purpose, grief and loss, trauma and intergenerational trauma
Child and Youth Mental Health and Addictions Services - Counselling and referrals for Indigenous children and youth * Arts-based activities and cultural teachings * Wholistic Knowledge Sharing Circle in partnership with Child and Parent Resource Institute * Outreach programs based on community consultation in London, Chippewas of the Thames, Munsee-Delaware First Nation, Oneida Nation of the Thames, Sarnia, Aamjiwnaang, Kettle and Stony Point, Moraviantown, and Caldwell First Nation
Clinical Services - Health care services for Indigenous people of all ages by Doctors and Nurse Practitioners * primary health care services to promote wellness, prevent illness, provide care for common concerns and manage ongoing chronic conditions * traditional Indigenous and western practices offered
Dental Clinic - available to adult clients with a status card or ODSP dental card as well as all patients age 0-17 * Call for details
Diabetes Education Services - Supports for people living with Type 2 diabetes, prediabetes or at high-risk for developing diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * diabetes nurse educator, registered practical nurse specializing in foot care and registered dietitian on staff * classes and workshops offered in some locations
FASD Education and Support - Nutrition and Fetal Alcohol Spectrum Disorder (FASD) education for individuals and families * FASD Community Support Group offered in London
Nutrition Services - Confidential nutrition counselling sessions to achieve and maintain health goals * topics include meal planning, budgeting, cholesterol and heart health, disease prevention, weight management, fiber and whole grain promotion, portion sizing and label reading, and prenatal nutrition
Palliative Care - Indigenous Palliative Care Team (IN-PaCT) supports people with palliative diagnoses in their place of choice * includes homes in London, Chippewas of the Thames, Oneida and Munsee-Delaware Nations * care team consists of a comfort care nurse practitioner, nurse, mental health therapist, and a traditional Indigenous healer/elder
Supporting Aboriginal Seniors at Home (SASH) - Serves patients 55 and older * primary health care services from a Nurse Practitioner for primary in clinic or home * Aboriginal Patient Navigator helps clients find way through complex health systems, identify barriers to care and providers referrals and linkages to other services * access to Senior's Health Advocate or Traditional Healing Coordinator to advocate for appropriate contemporary and traditional care and culturally safe care
Tele-Mental Health - access to specialized mental health consults for children and youth in rural, remote and under-served communities through videoconferencing * culturally-appropriate services
Traditional Healing - Indigenous healing methods to restore balance and connectedness * wholistic approach to individual, family, and community health * one-on-one sessions with Traditional Healer/Elder and Helper * group programs, including specialized cultural teachings, ceremonies or other learning events
