Service Description: Health centre focused on individual, family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * community development, health promotion and prevention services * referrals to other local and social health services * services aimed at people living with a low income, newcomers, individuals and families with chronic and complex illness

Community-led health teams develop programs to improve the social supports and conditions that affect long-term health
 
Health Outreach for People Experiencing Homelessness - Health, social services and daily programming for people who are experiencing or at risk of homelessness
