Service Description: Adult Protective Service Worker Program: Supports adults with a developmental disability who are living independently with advocacy, planning, case management and coordination

Passport Program - Supports for adults with a developmental disability to connect with their community and live as independently as possible * supports may include activities of daily living, community participation, funding for caregiver respite services and supports

Eligibility / Target Population: People with intellectual disabilities and their families * people 18 years of age and older or families with a child under the age off 18 with a developmental disability
