Service Description: Clients receive assistance in completing medical information in a provided template, so it is accessible for emergency personnel in the event of an emergency

Volunteers continue to follow-up with client every 3 months to ensure that the information remains up-to-date

Eligibility / Target Population: Adults 55 and older * people with disabilities, who need physical, social, or emotional supports
